Vertis Therapy is seeking a dedicated Physical Therapist to join our team for consistent PRN hours at our beautiful Assisted Living facility in Washington DC.
We are a therapist-owned company that provides ongoing training and support to our staff, ensuring our therapists have the resources they need to excel in their roles.
Key Responsibilities:- Evaluate and treat patients in accordance with physician orders, utilizing therapeutic procedures to address physical function deficits and improve independence, safety, and quality of life.
- Manage the delivery of care, ensuring quality and efficiency, and delegate appropriate components of care to Physical Therapist Assistants under supervision.
- Participate as a member of the interdisciplinary team, providing support and information within the area of physical therapy.
- Review patient information and evaluate its impact on the assessment and treatment process.
- Conduct and document thorough functional assessments of patients, including home/vocational and equipment needs.
- Develop and implement individualized treatment plans based on patient needs, containing achievable functional goals.
- Comply with evaluation, treatment, and documentation guidelines.
- Record patient progress in the patient record a minimum of every seven days.
- Revise treatment plans as needed throughout the treatment process.
- Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
- Instruct Assistants in treatment procedures and provide close supervision while they work with patients.
- Orient patients to program services and treatment procedures appropriate to their needs, informing them of any potential risks during procedures.
- Work with nursing staff, patients, and families to maximize patients' use of functional abilities.
- Establish and instruct patients and families/caregivers in exercises and therapeutic procedures to be continued following discharge.
- Assist in the referral process when patients require additional services following discharge, maintaining current knowledge of community resources.
- Attend and participate in rehabilitation team conferences regarding patient progress, problems, or needs.
- Attend and participate in other department/facility meetings as required.
- Access continuing education opportunities to maintain clinical competency.
- Participate in the quality improvement process by responding to results of medical record audits, patient/referral source satisfaction surveys, safety activities, and regulatory requirements.
- Master of Science degree in Physical Therapy or Bachelor of Science degree in Physical Therapy and five years of direct practice experience.
- Current license/certification in the state of practice.
- Prefer minimum one year experience in rehabilitation, supervision, training, and/or experience in a setting serving the same age/type of patient population served by this facility.
- Prefer membership in the American Physical Therapy Association.
- Demonstrated competency in identified areas of specialization.
- Excellent oral and written communication and interpersonal skills.
- Adequate computer skills to complete required job duties and computerized documentation.
- Demonstrated competency in applying principles, methods, materials, and equipment used in physical therapy.
- Knowledge of accreditation standards and compliance requirements.
