Position Overview

Woven Care is in search of a dedicated Outpatient Speech Language Pathologist to join our esteemed team.

Key Responsibilities
  • Collaboration: Work closely with families, healthcare providers, educational institutions, and our professional team to ensure optimal clinical outcomes and continuity of care.
  • Case Management: Engage in comprehensive case management activities adhering to departmental quality assurance and improvement standards.
  • Patient Care: Conduct thorough diagnostic evaluations in line with contemporary, evidence-based practices in speech-language pathology, integrating cultural competence and family-centered care principles.
  • Evidence-Based Practice: Deliver services that reflect current, evidence-based methodologies in speech-language pathology while meeting departmental quality assurance benchmarks.
Qualifications
  • Education: Master’s degree in Speech-Language Pathology.
  • Certifications: BLS for Healthcare Providers/CPR; ASHA Certificate of Clinical Competence (CCC) in Speech-Language Pathology.
  • Licensure: Valid Speech-Language Pathologist license from the State of Colorado, with credentialing completed within three months of employment.
Physical Requirements

This role necessitates the ability to sit for 1-2 hours and stand or walk for 4-6 hours daily. It may involve frequent bending and occasional lifting, carrying, pulling, pushing, and reaching. Precise motor skills with both hands are essential, and lifting up to 50 pounds may be required intermittently.
