About the Role

We are seeking a skilled and compassionate Registered Dialysis Nurse to join our team at Centers Dialysis Care. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the success of our dialysis center.

Responsibilities
  • Collaborate with the healthcare team to assess patient status and develop individualized care plans.
  • Perform care coordination and patient education to ensure optimal patient outcomes.
  • Administer medications and treatments as directed by physician staff.
  • Monitor patients throughout treatment and document their responses to treatment requirements.
  • Mobilize resources to maximize patients' control and participation in their recovery.
  • Integrate assessment and diagnostic information to anticipate patients' needs and provide proactive care.
  • Ensure seamless handoff communication and provide detailed reports on patients' situations and needs.
  • Develop and implement staffing plans to meet the needs of our patients.
  • Provide unique and individualized comfort measures using evidence-based practice.
  • Participate in performance improvement activities to enhance patient care and outcomes.
Requirements
  • Active NY RN License
  • CPR Certification
  • Clinical nursing experience in a dialysis setting is preferred
