MDS Coordinator

Courtyard Healthcare Center is seeking a skilled MDS Coordinator to join our team. As an MDS Coordinator, you will play a critical role in ensuring the accuracy and timeliness of patient assessments, collaborating with our nursing staff to provide high-quality care, and maintaining compliance with regulatory requirements.

• Coordinate and assist with the completion and submission of accurate and timely patient assessments, ensuring compliance with all applicable laws, regulations, and company policies.
• Report any changes in a patient's condition to the Director of Nursing (DON) and provide education to direct care associates regarding patient documentation, updates, and training on Skilled Nursing Facility and other Regulations.
• Ensure care plans are comprehensive, accurate, and meeting patient needs, reviewing Final Validation Reports to confirm assessments have been completed and accepted into the CMS and other systems as necessary and prior to billing.
• Collaborate with the Business Office regarding billing and continually review CMS Reports to address issues and improve processes to ensure that assessments are accurate and submitted on time.
• Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and perform functions of a staff nurse when necessary.
• Lead by example in treating everyone at work kindly and professionally.
