Position Overview:

We are seeking a Physical Therapist (PT) to provide specialized care to our patients.

Key Responsibilities:

  • Plan, deliver, and coordinate Physical Therapy services for patients.
  • Assess patients' physical capabilities and collaborate with them and their families to provide training and assistive devices to address deficits related to gross motor skills.
  • Collaborate with interdisciplinary team members to set rehabilitation and maintenance objectives.

Essential Duties:

  • Evaluate patients' Physical Therapy needs and deliver services based on authorized medical directives.
  • Conduct clinical assessments to identify functional limitations and potential for recovery, developing a tailored Plan of Care with specific goals.
  • Educate patients, families, caregivers, and health team members on the use of assistive devices and appropriate therapy techniques.
  • Monitor and document patients' responses to treatment, reporting any changes to the medical team.
  • Work collaboratively with the healthcare team to expedite patient recovery and manage any residual disabilities.
  • Communicate bi-weekly with nursing staff to review patient progress and address any service-related issues.
  • Reassess patients as necessary to track progress and update treatment plans accordingly.
  • Identify and report any operational or safety concerns regarding equipment used in patient care.
  • Prepare discharge summaries upon completion of services, detailing care outcomes and recommendations for follow-up.

Qualifications:

  • Graduation from a Physical Therapy program accredited by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.

Experience and Skills:

  • Minimum of one year of experience as a Physical Therapist in an acute care environment; two years preferred, with home care experience being advantageous.
  • Strong knowledge of current Physical Therapy standards, procedures, and techniques.
  • Exceptional assessment and technical skills.
  • Organized and culturally sensitive.

Licensure:

  • Current Physical Therapist license in New York State is required.
