Job Title: Registered Nurse

LHC Group is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes. As a Registered Nurse, you will be responsible for delivering comprehensive nursing care, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless patient care.

About the Role:
  • Provide direct patient care, including assessments, treatments, and education
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care
  • Conduct regular patient assessments to evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation
