Job Summary:The successful candidate will provide high-quality patient care, including assessment, treatment, and charting. They will work collaboratively with our medical team to develop care plans and implement programs and procedures.
Responsibilities:- Collects data and adjusts processes to accommodate program requirements.
- Maintains licensure and certification as required and participates in ongoing education activities.
- Teaches and supports patients with self-care management.
- Actively seeks input to support patient care and outcomes.
- Participates in program evaluation activities.
- Follows up on abnormal laboratory results requiring patient action and possibly re-measurement.
- Handles requests for medication refills according to ACP procedure.
- Coordinates outpatient work-up and or treatment plans when patient situation requires it.
- Provides direct care for patients needing blood pressure check, wound check, technique assessment.
- Monitors patients while awaiting transport to Emergency Room and assures documentation is complete.
- Participates in the development of strategies to maintain high levels of wellness for at-risk patients.
- Completion of nursing training from an accredited school of nursing.
- Minimum of one to two years' experience working in a Medical Practice/Primary Care, Med Surg, ER and/or Occupational Health.
- Current New York State Registered Nurse license.
