Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Spaulding Rehabilitation. As a key member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to provide comprehensive social work services.

Key Responsibilities
  • Conduct thorough clinical social work assessments to identify patient and family needs, and develop effective intervention plans to support their well-being.
  • Collaborate with the healthcare team to formulate treatment plans that address patients' psychosocial needs and promote optimal outcomes.
  • Provide education and support to patients and families on health care and community resources, empowering them to make informed decisions about their care.
  • Develop and maintain relationships with community and social service agencies to access resources that benefit patients and families.
  • Teach and mentor new staff and healthcare students to ensure they meet performance expectations.
  • Deliver educational in-services and case conference presentations to integrate best practices into service.
  • Provide professional consultation to healthcare professionals and hospital personnel to effectively manage complex patient/family or interpersonal situations.
  • Assess and report suspected abuse, neglect, and domestic violence in compliance with regulatory requirements.
  • Document patient information according to department, hospital, and regulatory standards.
  • Identify performance improvement opportunities and implement action plans to enhance patient care and outcomes.
  • Align patient care with hospital length of stay and financial reimbursement goals.
  • Complete documentation to support third-party payer requirements.
  • Collaborate with program leadership to improve program design, structure, and resources to achieve optimal patient outcomes.
  • Conduct research to advance best practices in social work.
Qualifications
  • Masters degree in Social Work from an accredited school of social work.
  • Current Massachusetts license as LCSW or LICSW.
  • One to two years of clinical experience in a medical setting with a mental health background preferred.
Skills
  • Computer proficiency required, with Microsoft office applications preferred.
  • Highly developed communications and interpersonal skills, working with diverse populations.
  • Ability to work independently, be self-directed, and contribute as a member of a team.
  • Anticipation of challenges and development of strategies to address them.
  • High level of service delivery, with ability to prioritize work, meet deadlines, and adapt to changing situations.
  • Attention to detail.
