Job Title: Clinical Pharmacist

Option Care Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing safe and effective pharmacy services to our patients in the comfort of their own homes.

Key Responsibilities:
  • Assess patients' suitability for home care services and design individualized pharmacy care plans
  • Monitor and evaluate patient progress, making recommendations for modifications in medication therapy as needed
  • Provide medication-use education to patients, caregivers, and healthcare professionals
  • Ensure continuity of pharmaceutical care across all patient-care settings
  • Manage and troubleshoot medication administration equipment and related systems
Requirements:
  • Graduate of an accredited School of Pharmacy
  • Current, active Pharmacy license as granted by the State Board of Pharmacy
  • At least one year of experience in sterile IV admixture procedures or equivalent experience
Preferred Qualifications:
  • Completion of an ASHP accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience
  • Experience in home care, hospital, or other clean room settings
