Join Our Care Team as a Part-Time Hospice Registered Nurse

We are seeking a compassionate and experienced Registered Nurse to join our team at Residential Home Health and Hospice. As a Part-Time Hospice RN, you will play a vital role in providing high-quality patient care and support to individuals in need.

About the Role

This part-time position is ideal for a nurse who wants to make a difference in the lives of others while maintaining a healthy work-life balance. You will work on Saturdays and Sundays, 6:30am - 6:30pm, providing care to patients in the Troy area.

Responsibilities

  • Provide patient care and support in a team-based environment
  • Collaborate with other healthcare professionals to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized care plans to meet patient goals and objectives
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements

To be considered for this role, you must have:

  • A current RN license in the state where you will be working
  • An Associate degree or higher from an accredited School of Nursing
  • At least two years of RN experience, with hospice experience preferred
  • A valid driver's license and ability to spend approximately 20% of your day driving to and from patient locations
  • The skills and ability to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
