Home Health Speech Language Pathologist Position

We are seeking a skilled Speech Language Pathologist to join our team at Alternate Solutions Health Network. Our clinical teams provide compassionate quality care and produce positive outcomes for our patient population.

You will provide home-based skilled speech therapy care for patients as directed by the attending physician. Your role will involve guiding patients in their use of therapeutic, creative, and self-care activities to improve functioning.

Key responsibilities include observing, recording, and reporting patient reactions, changes, and progress to the attending physician and any appropriate agency staff. You will also use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or any other speech therapy-related function.

Patient Outcomes:

  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or any other speech therapy-related function.
  • Help decrease re-hospitalizations by teaching patients on preventative measures and making good decisions.

Family Educator/Advocate:

  • Counsel patients and families on the disease/injury and how to manage in all phases of speech therapy.
  • Counsel and instruct patients and families on the disease/injury/disability and how to manage in all phases of speech therapy.

Operations:

  • Attend in-service training and mandatory agency meetings.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
