Care Team Program Lead

Job Description
The Program Manager will oversee the daily operations of the residential care team, ensuring that all aspects of care are met. This includes staff development and supervision, staffing oversight, leadership, medical care coordination, direct support, facility management, and service coordination.
Responsibilities
- Staff Development and Supervision: Train and mentor DSPs to ensure effective support of the individual.
- Staffing Oversight: Oversee DSP staffing schedules, approve time off requests, and arrange coverage when staff call out.
- Leadership: Ensure the team functions effectively and collaboratively, set a leadership example by demonstrating a continuous learning mindset, respect, and commitment to excellence.
- Medical Care Coordination: Attend medical appointments, perform special care procedures, update Medication Administration Records, and maintain strong communication with agency nurse and medical providers.
- Direct Support: Serve as a regularly scheduled second DSP, provide substitute staff coverage, and provide personal care including meal preparation and assistance with activities of daily living.
- Facility Management: Ensure team members maintain the site properly, address or report maintenance concerns, and maintain the site with needed groceries and supplies.
- Service Coordination: Perform service coordination duties for the individual served, including plan writing, data collection, and review.
Requirements
- High School diploma or equivalent; BA or BS preferred.
- Demonstrated supervisory skills.
- Ability to lead a team that demonstrates a high degree of collegiality and collaboration.
- Valid Drivers License, acceptable driving record, and ability to drive a van.
- Ability to provide physical support in shallow water, walk up to 45 minutes across ordinary terrain, and maintain confidentiality.
- BASIC COMPUTER SKILLS SUFFICIENT TO COMPLETE WORK-RELATED DOCUMENTATION, COMMUNICATE WITH COWORKERS, AND SUBMIT TIMESHEETS.
