About the Role

We are seeking an experienced RN to join our team as a Care Advisor. In this role, you will work with patients, physicians, and care teams to create and implement care plans that address identified needs.

Key Responsibilities
  • Assess patient's knowledge of their clinical condition(s) and provide education and self-management support based on the patient's unique learning style.
  • Measure, improve, and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
  • Perform ongoing education to patient and patient's family and/or caregiver on diagnosis, prognosis, treatment, and medications.
  • Serve as advocate when problems and issues arise.
Requirements
  • Education: Bachelor's of Nursing Degree (BSN)
  • Experience: 2 years' RN experience - RN Case management, Critical Care RN, or social work highly preferred.
  • Licenses/Certifications: Current RN License in the State of Practice
