Job Summary

Bear Mountain Healthcare is seeking a highly skilled Physical Therapist to join our in-house rehabilitation team. As a Per Diem Physical Therapist, you will be responsible for planning, organizing, and directing Physical Therapy services in accordance with current federal and state standards, guidelines, and regulations.

The ideal candidate will have a strong background in geriatric care and be able to evaluate patients' conditions and devise customized rehabilitation and treatment plans to maximize quality of life.

Responsibilities
  • Plan, develop, and implement Physical Therapy services and programs in accordance with current regulations and guidelines.
  • Maintain accurate documentation, department records, and reports.
  • Communicate effectively with residents, family members, and other team members regarding PT programs and evaluation findings.
  • Initiate referrals as necessary and in accordance with Physical Therapy Department procedures.
  • Conduct regular screenings of residents to determine need for intervention/treatment.
Requirements
  • Active state license or license eligible.
  • Bachelor of Science, Master of Science, or Doctorate in Physical Therapy from an accredited program.
