We are seeking a compassionate and experienced Licensed Assessment Therapist to join our team at Preferred Family Healthcare. In this rewarding role, you will be responsible for assessing, diagnosing, and treating clients with mental health and/or substance use disorders using evidence-based interventions and techniques.
Key Responsibilities:- Provide individual, group, family, and/or other practical counseling in a variety of settings to meet the needs of the client/patient and their goals of treatment
- Develop treatment plans for clients/patients in accordance with state regulatory/accreditation/agency standards that demonstrate individualization and evolution based on information gathered from assessment
- Participate in clinical staffing/treatment meetings, providing feedback on all pertinent cases
- Meet department-specific Collaborative Service Goal (CSG) requirements
- Document all services provided in accordance with state regulatory/accreditation/agency standards
- Ensure that services are entered within the appropriate timeframe, appropriate format, notes reference treatment plans, etc.
- Correct any file/charting deficiencies within the appropriate timeframe
- Assist clients/patients through the transition process and complete final correspondence such as transition summary, plan, letter, etc.
- Facilitate group counseling, group education, family therapy, and individual sessions including specialty services such as trauma and co-occurring therapy
- Other duties as assigned by leadership
- Must possess at least a master's degree in Social Work, Psychology, Counseling or related field
- Holds licensure with the applicable board of professional registration for the state that they are practicing as a professional counselor or social worker
- Must be willing to become licensed in additional states, other than the state currently licensed in
- Must be willing and comfortable with providing telehealth services to the individuals we serve (training will be provided)
- Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT)
