Job Summary:

We are seeking a highly skilled and dedicated Physical Therapist to join our team at Delhi Rehabilitation. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our rehabilitation services.

Key Responsibilities:

• Screen residents for rehabilitation therapy needs and develop individualized treatment plans
• Conduct physical therapy evaluations and assessments to identify areas for improvement
• Establish and implement physical therapy plans of care, including modalities and treatments
• Modify treatment programs as needed to ensure optimal patient outcomes
• Supervise physical therapy assistants and unlicensed assistive personnel to ensure high-quality care
• Participate in interdisciplinary team meetings to discuss patient progress and develop care plans
• Maintain accurate and detailed records of patient treatment and progress
• Collaborate with other healthcare professionals to ensure seamless patient care

Requirements:

• Current unencumbered NYS license as a Registered Professional Physical Therapist
• Bachelor's degree or higher in Physical Therapy or related field
• Geriatric experience preferred
• Ability to communicate effectively with patients, families, and healthcare teams
• Strong problem-solving and critical thinking skills
• Ability to work independently and as part of a team
