Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our Behavioral Health Department at the Chinatown Service Center. The successful candidate will be responsible for coordinating and following up on care plans under the direction of a clinical provider or in relation to program services.

Key Responsibilities
  • Conduct initial client pre-screening to determine service eligibility and appropriateness of case management.
  • Link clients with healthcare, psychosocial, and other services.
  • Verify and support enrollment in medical care, particularly for uninsured clients.
  • Perform client intake, needs assessment, and required paperwork completion.
  • Collect core data elements for database input.
  • Develop comprehensive care plans jointly with clients, addressing short and long-term health goals.
  • Provide referrals to community agencies and assist with transportation arrangements and appointment reminders.
Requirements
  • Bachelor's degree required in a healthcare, social services, human services, or gerontology-related field.
  • At least one year of experience in a behavioral health-related field, two years of case management-specific experience preferred.
  • Bilingual/bicultural English and Chinese (Cantonese and/or Mandarin) preferred.
  • Ability to display sensitivity to all cultures and knowledge of principles and techniques of social, cultural, and intergeneration dynamics of Chinese/Asian immigrants and refugees.
  • Self-motivated, self-driven, and able to work independently within the scope of assigned tasks with minimum supervision.
  • Resourceful and flexible in response to changing environments and needs.
  • Customer service skills with excellent communication and interpersonal skills, both in writing and verbal, to provide support in-person, via telephone, email, and web-based correspondence.
  • Proficient in word processing and data entry using Macintosh, Microsoft Word, Excel, and Outlook.
  • Reliable transportation.
