We are seeking a skilled Registered Dietitian to join our team at Weinberg Campus. As a key member of our healthcare team, you will conduct assessments of our skilled nursing residents, assess their nutritional needs, and implement care plans that meet their diagnoses and dietary requirements.

• Conduct initial and periodic nutrition assessments on assigned clients, adhering to developed standards and evidence-based practice.
• Develop and implement client-specific nutrition care plans, communicating them to the interdisciplinary team and other individuals with a need-to-know.
• Provide nutrition education and guidance to clients and caregivers, conducting patient education/teaching in areas of enteral nutrition.
• Monitor special diets and nutritional non-compliance, providing updates on patient status and the effectiveness of care plans to the Director of Food Services, RN case managers, and the interdisciplinary team.
• Document assessments and dietitian notes in the clinical record.

• High school diploma required; Bachelor's degree from an accredited college with major studies in food and nutrition or dietetics required.
• Credentialed with Commission on Dietetic Registration.
• Ability to work with clients in their homes and lift 25 pounds.
