Job Summary:
Bethesda Health is seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. This rewarding role offers the opportunity to make a meaningful impact in the lives of our residents while advancing your nursing career. As a key member of our interdisciplinary team, you will be responsible for assessing residents' health status, developing and implementing care plans, and ensuring safe and timely nursing interventions.

About the Role:
This is a full-time position offering an estimated salary range of $83,000 - $110,000 annually, depending on experience and qualifications. In this role, you will work closely with our residents, families, and healthcare team to provide comprehensive care that meets their unique needs and preferences. Your strong communication skills, compassion, and critical thinking abilities will enable you to build trust and rapport with our residents and their loved ones.

Responsibilities:
- Assess residents' health status and develop individualized care plans
- Collaborate with the healthcare team to ensure timely and effective nursing interventions
- Maintain accurate and up-to-date documentation of resident care and progress
- Communicate effectively with residents, families, and healthcare professionals to address concerns and provide updates
- Participate in quality improvement initiatives to enhance the delivery of care

Requirements:
- Current RN licensure in the state of practice
- At least 2 years of experience in long-term care or a related field
- Strong communication and interpersonal skills
- Ability to lift 50 pounds and assist with transfers as needed
- Manual dexterity and ability to use electronic medical records software
- Registered Nurse (RN) certification preferred but not required
