About the Role

We are seeking a skilled Registered Nurse to join our Home Health and Hospice team at Duke Health. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.

Responsibilities
  1. Assess patient needs and develop individualized care plans to meet their unique needs and goals.
  2. Provide direct patient care, including wound management, medication administration, and education on disease management and self-care.
  3. Collaborate with healthcare team members to ensure seamless care coordination and communication.
  4. Document patient progress and outcomes in accordance with regulatory requirements and agency policies.
  5. Participate in ongoing education and professional development to stay current on best practices and evidence-based care.
Requirements
  • Current RN licensure in North Carolina or compact state.
  • 2+ years of experience in home health, hospice, or med-surg nursing.
  • Valid driver's license and reliable transportation.
  • Ability to work autonomously and as part of a team.
