About the Role:

  • Provide ongoing medical coverage to participants, including assessments and care planning
  • Collaborate with community-based physicians and healthcare teams in discharge planning
  • Participate in IDT meetings, communicating participant updates, making care decisions, and coordinating 24-hour care delivery
  • Oversee care of participants in nursing homes, conducting regular visits as needed
  • Participate in rotating night and weekend call schedules

Requirements:

  • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
  • Doctorate degree in medicine
  • Board certification in Internal Medicine, Family Practice, or Geriatrics (preferred)
  • Active, unrestricted California medical license
  • Current state CDS certification and DEA registration
  • Eligible for enrollment to provide services to Medicare and Medi-Cal participants
