The role involves delivering hospital case management services, which encompass:
- Utilization management and discharge planning for assigned patients, ensuring efficient and cost-effective patient progression from admission to discharge.
- Collaboration with social services to facilitate optimal post-acute care services.
- Application of established clinical guidelines to support appropriate levels of care and medical necessity throughout the hospitalization process.
- Coordination with a multidisciplinary team, including physicians, staff, and payers, to guarantee the efficient delivery of quality, cost-effective care.
- Ensuring compliance with state and federal regulatory standards and adherence to the Canyon Vista Medical Center Utilization Management Plan.
- Associate's Degree in Nursing is required; Bachelor's and/or Master's degree is preferred.
- A minimum of 5 years of experience in an Acute Care setting is preferred.
- Experience in Discharge Planning is preferred.
