Key Responsibilities:

• Conduct comprehensive in-home assessments, including medical history reviews, physical exams, and vital sign checks.
• Perform evidence-based practice screenings, such as point-of-care testing, to identify potential health risks.
• Develop personalized care plans to address patients' unique needs and health goals.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
• Travel to patients' homes, navigating various dwelling conditions and environments.

Requirements:

• Active NP or PA license, with the ability to obtain national certification in state of assignment.
• 1+ years of clinical experience in a relevant setting, such as gerontology, cardiology, or internal medicine.
• Ability to work independently, with minimal supervision, and as part of a collaborative team.
• Reliable transportation and a valid driver's license.
