Job Summary

We are seeking a licensed practical nurse (LPN) to join our team of caregivers in providing meaningful patient experiences. As a hospice LPN, you will work under the direction of a registered nurse to conduct patient visits and provide routine care, while communicating with the care team, patient, and family.

Responsibilities
  • Conduct patient visits and provide routine care under the direction of a registered nurse
  • Communicate with the care team, patient, and family to ensure seamless care
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans
  • Collaborate with other healthcare professionals to provide comprehensive care
  • Document patient information and care provided in a timely and accurate manner
Requirements
  • Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
  • 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
  • Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
