Job Summary:
The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. This role requires a collaborative approach to anticipate, integrate, and coordinate patient care plans to maximize the ability to remain in a community setting.

Key Responsibilities:
• Identify at-risk patient populations who require care management services
• Collaborate with physicians to develop patient care plans
• Serve as a patient/family advocate and communication link between the healthcare team and community resources
• Support the mission, values, and management of patient care services

Requirements:
• 2 years of nursing experience or no experience required with preferred education
• Diploma or Associate's degree in nursing required; Bachelor's degree preferred
• Registered Nurse licensure required

Work Schedule:
Every other weekend, 8am-8pm

Weekend and Call Requirements:
Every other weekend; no call requirements
