Key Responsibilities
  • Conduct thorough patient assessments to identify needs and develop effective care plans
  • Provide skilled nursing care, including wound care, medication management, and patient education
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions
  • Communicate effectively with patients, families, and healthcare providers to ensure optimal outcomes
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Current RN license in the state of practice
  • Basic Life Support (BLS) certification for healthcare providers
  • Valid driver's license, insured vehicle, and reliable transportation
  • Ability to communicate effectively verbally and in writing
Preferred Qualifications
  • Bachelor's Degree in Nursing (BSN)
  • Two years of experience as an RN
  • Previous experience in an acute care, home health, or hospice setting
Physical Requirements

The ideal candidate will possess the following physical abilities:

  • Ability to see and read information, labels, and patient charts
  • Ability to hear and understand spoken information, alarms, and patient needs
  • Manual dexterity to manipulate complex equipment and supplies
  • Ability to lift and transfer patients, as well as push/pull heavy equipment
  • Ability to navigate crowded and busy environments
