Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at SilverCrest, an Assisted Living Facility. As an LPN, you will play a vital role in providing high-quality care to our residents.

Key Responsibilities:

  • Develop and implement individualized care plans to meet the unique needs of our residents
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication
  • Conduct thorough assessments and evaluations to identify resident needs and preferences
  • Document resident progress and outcomes in a timely and accurate manner

Requirements:

  • Current Minnesota nursing license
  • At least one year of LPN experience, preferably in assisted living or similar settings
  • CPR Certification required
