Job Summary:

ServiceNet is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in helping clients facing persistent mental health and substance use challenges to live joyful and fulfilling lives. Your role will involve providing life-changing care coordination, coordinating care with mental health and healthcare providers, and assisting clients in navigating services such as legal aid, housing, employment, substance use treatment, and medical care.

Key Responsibilities:

Provide care coordination for clients with persistent mental health and substance use challenges.

Coordinate care with mental health and healthcare providers to create a seamless, integrated care experience.

Assist clients in navigating services such as legal aid, housing, employment, substance use treatment, and medical care.

Collaborate with your team to ensure each client receives the support they need to thrive.

Build partnerships within the community to make it easier for clients to access the resources they need.

Qualifications:

A BA/BS in Social Work or a related field.

At least 3 years of experience in care coordination.

Compassionate, excellent communication skills, and a willingness to make a difference.

Ability to travel, with a valid driver's license and reliable vehicle.
