We are seeking a skilled Care Coordinator to join our team at Optum Home & Community Care. As a key member of our team, you will play a vital role in delivering high-quality care to our patients.
Key Responsibilities:- Conduct in-home assessments to evaluate patient needs and develop personalized care plans.
- Perform evidence-based practice screenings, including point-of-care testing, to identify potential health risks.
- Collaborate with healthcare providers to ensure seamless care coordination and communication.
- Identify and address gaps in care, ensuring patients receive comprehensive and coordinated care.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
- Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
