Key Responsibilities:
  • Conduct in-home assessments to evaluate patient needs and develop personalized care plans.
  • Perform evidence-based practice screenings, including point-of-care testing, to identify potential health risks.
  • Collaborate with healthcare providers to ensure seamless care coordination and communication.

Requirements:
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
  • Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
