Pathways Nursing and Rehabilitation Center is seeking a skilled Nutrition Specialist to provide high-quality patient care in Niskayuna, New York. As a leading complex care nursing home, we offer a modern work environment and a team-oriented approach to care.

Responsibilities:

  • Assess and document residents' nutritional status, developing personalized care plans based on evidence-based practices.
  • Consult with physicians to ensure accurate nutrition and hydration plans, and provide updates on progress.
  • Counsel clients and families on special diets and nutritional health, both in-house and for discharge to a home setting.
  • Participate in departmental continuous Quality Improvement implementation and monitoring, ensuring the highest standards of care.

Requirements:

  • Must possess and maintain Registered Dietitian status through the Commission on Dietetic Registration.
  • 2 years of experience preferred.
  • Ability to read, write, speak, and understand the English language.
  • Knowledge of dietary practices and procedures, as well as laws, regulations, and guidelines governing dietary functions in long-term care facilities.
