About Hopscotch Primary Care
At Hopscotch Primary Care, we are dedicated to ensuring that exceptional healthcare is accessible to all individuals within every community.

Currently, nearly 20% of Americans reside in rural areas, yet only 11% of physicians are found practicing in these regions. We are committed to revolutionizing healthcare in rural America.

Our mission is to deliver high-quality primary care that is specifically designed to address the unique needs of our patients through our comprehensive care model and an extensive care team. We provide care not only in our clinics but also across various settings, ensuring that resources are available to those who require them the most.

At Hopscotch, our patients and the dedicated teams that serve them are at the heart of everything we do. We adopt a collaborative approach to meet patient needs and provide the highest standard of care.

Our Vision
We aim to deliver the kind of care that we would wish for ourselves or our loved ones, ensuring it is provided in the appropriate setting and at the right time.

Currently, we are serving thousands of patients through our value-based care model, and this number continues to grow.

Your Role as Medical Director
As the Medical Director for our clinic, you will be responsible for providing daily leadership to the clinic teams, ensuring the delivery of high-quality care, an outstanding patient experience, and fostering a top-tier culture. • Promote the Hopscotch Care Model within the community, advocating for enhanced care for Medicare patients.
  • Proactively manage operational needs in collaboration with the Care Center Manager, ensuring continuous patient care.
  • Guide the care team by exemplifying best practices in workflow efficiency and documentation.
  • Participate actively in Clinical Forums, providing insights and ensuring effective communication of key points to your clinic team.
  • Mentor and coach providers to support their integration and operational excellence within the clinic.
  • Collaborate with the Care Center Manager to exceed target metrics related to patient experience and care coordination.
  • Deliver comprehensive primary care services to a dedicated patient panel, including both in-person and telehealth visits.
  • Build trust with patients and caregivers, engaging them in their care plans and wellness strategies.
  • Foster a culture of high performance and continuous improvement, emphasizing quality and patient-centered care.
  • Represent Hopscotch Health in the community through education and engagement initiatives.
