Job Summary
We are seeking an experienced Physical Therapy Assistant to join our multidisciplinary team. As a Physical Therapy Assistant at Liberty Healthcare, you will have the opportunity to work with a dynamic team to provide exceptional care and rehabilitation services to our patients.
Key Responsibilities
- Work collaboratively with our physical therapists to develop and implement individualized treatment plans for patients.
- Provide direct patient care, including exercises, modalities, and other interventions as prescribed by the physical therapist.
- Conduct patient assessments and evaluations to identify areas of improvement and develop strategies for rehabilitation.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
- Graduate of an accredited physical therapy assistant program.
- Licensed in the state where the SNF is located.
- Previous experience in a rehabilitation setting, preferably in a skilled nursing facility.
