- Document patient progress, communicate with physicians and other healthcare professionals, and maintain accurate records.
- Supervise and educate home health aides and personal care workers to ensure high-quality care.
- Discharge patients after consultation with physicians and clinical managers, ensuring a smooth transition to other care settings.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum one year of nursing experience, preferably in home health or a related field.
- Strong med-surg, ICU, ER, and acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
