In your role as the RN Case Manager, you will take full nursing responsibility for implementing the Plan of Care for each hospice patient, while continuously assessing both personal and professional performance to enhance productivity and the quality of care provided.
- Conduct the initial nursing assessment to determine eligibility for hospice services during a visit within forty-eight (48) hours of referral.
- Assess the physical, psychosocial, and environmental needs of the patient and family, with re-evaluations occurring no less than every fifteen (15) days.
- Initiate and coordinate the care plan while maintaining precise and relevant clinical documentation regarding the patient's condition.
- Record issues, set appropriate goals, outline interventions, and document patient/family responses to hospice care.
- Collaborate daily with the patient/family, attending physician, and other members of the interdisciplinary group (IDG) to provide comprehensive care.
Experience Requirements:
- A minimum of one year of clinical experience.
License Requirements:
- Current RN licensure in the state of practice along with one year of clinical experience.
- Current CPR Certification.
- A valid driver's license, vehicle insurance, and access to reliable transportation, or public transit.
