Job Summary:

The LPN will provide nursing supports to individuals with intellectual disabilities, working under the supervision of a Registered Nurse. The ideal candidate will have a strong background in delivering healthcare services and a passion for working with individuals with special needs.

Responsibilities:
  • Provide nursing supports to individuals with intellectual disabilities, including assisting with direct care and reviewing medical records.
  • Monitor individuals' medical needs and health status, provide treatments and care, and document medical histories.
  • Ensure that individuals' prescriptions are filled/refilled, record physician's medication orders, and monitor medication administration sheets.
  • Review clinical data and upload medical information in My Health and update Evero.
  • Complete and provide weekly reports to the IDT team.
Qualifications:
  • Associate degree in Nursing and two years of professional experience in delivering healthcare services to individuals with intellectual disabilities.
  • Licensed as a Licensed Practical Nurse required.
  • Excellent presentation and writing skills.
  • Knowledgeable in Electronic Health Records.
  • Valid Driver's License for travel to multiple locations.
