We are seeking a compassionate and friendly Family Nurse Practitioner to join our team at Unicare Community Health Center in Fontana.
About the Role:
- The Family Nurse Practitioner will provide consultation, diagnosis, and treatment of related medical conditions under the supervision of the Chief Medical Officer.
- The ideal candidate will have a minimum of two years of clinical experience and excellent written and verbal communication skills.
- Knowledge of Electronic Health Records (EHR) and Microsoft Office is required.
- The successful candidate will thrive in a fast-paced environment and be able to work independently.
- A thorough knowledge of principles and practices of modern medicine related to public health services is essential.
- Demonstrated ability to work effectively with diverse individuals in a multicultural environment is also required.
Responsibilities:
- Examine patients, take medical histories, record results, and make preliminary diagnoses or decide on follow-up procedures.
- Administer or order diagnostic tests, blood work, and interpret test results.
- Perform therapeutic procedures such as immunizations, injections, and managing infections.
- Exercise professional judgment regarding consultation with the medical director concerning appropriate treatment.
- Develop and implement patient care plans, instruct, and counsel patients, and record progress.
- Pursue DEA Certification and AANP certification as per company guidelines.
- Prescribe medications within the scope of practice allowed by state guidelines and clinic regulations.
- Provide health education on health maintenance, especially in the realm of STD prevention.
- Prescreen cases requiring review by the Medical Director.
- Participate in staff meetings, planning meetings, and other meetings as needed.
- Contribute to ongoing quality assurance monitoring.
