WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) offers seniors the chance to maintain their independence while receiving comprehensive care within their communities.
Our dedicated approach to providing a wide array of medical services to our participants fuels our commitment to treating the entire individual rather than merely addressing symptoms. The Primary Care Physician conducts thorough in-person evaluations and collaborates with the Interdisciplinary Team (IDT) to formulate personalized care plans for each participant.
Under the guidance of the Medical Director, the Primary Care Physician supervises all aspects of participant care in various settings, including nursing facilities and hospitals, ensuring seamless admissions and discharges for both acute and long-term care while coordinating with medical specialists for optimal care management.
Additionally, the Primary Care Physician provides leadership and guidance to Nurse Practitioners and other IDT members.
Key Responsibilities:
- Deliver comprehensive assessment, planning, primary care, and ongoing medical support to participants.
- Collaborate with community physicians and the IDT in planning discharges from acute or long-term care environments.
- Engage actively in IDT meetings, communicating changes in participant conditions, contributing to care planning, and ensuring 24-hour care coordination.
- Regularly visit nursing home participants based on their individual needs.
- Participate in a rotating schedule for night and weekend on-call duties.
Qualifications:
- Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Board Certification in Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics (preferred).
- State CDS Certification and DEA Registration.
