Responsibilities:
Provide safe and effective care to patients with complex medical conditions
Collaborate with interdisciplinary teams to develop and implement individualized care plans
Utilize the Synergy Model for Care to personalize patient care and improve outcomes
Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Document patient care accurately and in a timely manner
Requirements:
Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board
Graduate of an accredited school of nursing with 12 months of recent nursing experience within the past five years or recent completion of a re-entry nursing program
BLS certification required, ACLS may be required in designated departments
Physical and Environmental Demands:
Medium-Heavy lifting (occasionally); 20-35 lbs, 34-66% of the workday (frequently); 10-20 lbs, 67-100% of the workday (constantly)
Occasional to frequent standing and walking; occasional sitting
Close eye work (computers, typing, reading, writing)
Environmental exposures include, but are not limited to, blood-borne pathogen exposure, bio-hazardous waste, chemicals/gases/fumes/vapors, communicable diseases, electrical shock, floor surfaces, hot/cold temperatures, indoor/outdoor conditions, latex, lighting, patient care/handling injuries, radiation, shift work, travel may be required, use of personal protective equipment, including respirators
