About the Role

Gentiva, a leading provider of hospice and palliative care services, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Per Diem Hospice LPN. In this role, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Provide direct patient care services, including administering medications, wound care, and other treatments as prescribed by the physician.
  • Evaluate patient needs and develop individualized care plans to ensure optimal comfort and quality of life.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and report any changes or concerns to the interdisciplinary team.
  • Participate in staff meetings and education sessions to stay up-to-date on best practices and industry developments.
Requirements
  • Current LPN license to practice in the state of employment.
  • 1+ years of experience in hospice, home health, ICU, geriatric, or oncology care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Current CPR certification.
  • Valid driver's license and automobile insurance.
