Compassionate Patient Care Coordinator

We are seeking a skilled and compassionate Registered Nurse to join our team as a Compassionate Patient Care Coordinator.
The estimated salary for this position is $80,000 - $100,000 per year, depending on experience and qualifications.
Job DescriptionAs a Compassionate Patient Care Coordinator, you will be responsible for assessing patients' conditions, charting observations and evaluations, and administering medication as prescribed by the patient's physician.
You will also educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient's quality of life.
The ability to work effectively with patients, family members, team members, and other healthcare professionals is essential for this role.
Qualifications- Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing
- Current state license as an RN
- Proof of current CPR certification
- Compassionate and friendly demeanor
- Excellent communication skills
- Current driver's license and reliable transportation
- Bilingual preferred
