About the Position:

We are seeking a skilled LPN to lead the Wellness departments at Grace Park, a 92-bed personal care home. This is a rewarding opportunity to make a difference in the lives of our residents and their families.

The estimated annual salary for this role is $70,000-$90,000, depending on experience.

Key Responsibilities:

  • Develop and implement strategies to improve resident outcomes and satisfaction
  • Collaborate with interdisciplinary teams to ensure comprehensive care planning
  • Mentor and educate staff members to enhance their skills and knowledge

Requirements:

  • Current LPN licensure in the Commonwealth of Pennsylvania
  • CPR certification required
  • Minimum of 3 years experience working as an LPN, preferably within the aging services industry
  • Proven leadership and coaching skills
