Job Summary

The Advanced Practice Clinician will provide holistic, compassionate care to Medicare Advantage plan members and families. This role involves collaborating with physicians, Care Coordinators, facility staff, family members, and other healthcare professionals to diagnose, manage, and prevent acute and chronic illness and disease, and promote wellness.

Key Responsibilities

• Perform in-depth health risk assessments on assigned plan members
• Work collaboratively with physicians, Care Coordinators, facility staff, family members, and other healthcare professionals as needed
• Identify quality improvement activities through assessing and planning healthcare that is individualized to the member and consistent with policies and protocols
• Review and interpret medical, surgical, and social history of members
• Perform appropriate screening procedures for members
• Interpret laboratory and other diagnostic data and recommend appropriate treatments or actions for follow-up, reconcile medications
• Prescribe or perform various treatment modalities, including medications, injections, wellness counseling, suturing, and ancillary testing
• Comply with OSHA, State Board of Nursing, and other regulatory requirements
• Participate in mandatory compliance and other training programs, seek guidance for compliance-related concerns, and adhere to all applicable laws, regulations, policies, and procedures
• Refer members to PCP, Care Management, and Disease Management programs as necessary
• Practice in accordance with applicable local, state, and federal laws governing confidentiality and medical information privacy regulations (HIPAA)
• Maintain control of all PHI information
• Provide constructive and timely feedback about recommendations to improve and/or streamline operations
• Maintain a focus on timely customer service
• Perform direct Care Management Support as needed
• Synthesize information and easily diagnose and participate in the development of the Plan of Care
• Create positive rapport and engagement with plan members
• Assist on special projects as assigned
• Participate in client meetings as requested
• Communicate and collaborate with Facility leadership to achieve Member care needs and goals

Qualifications

• Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field
• Master's Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team
