Job Summary:

The Admissions and Care Coordinator will provide and arrange for hospice care for patients in a collaborative relationship with the Interdisciplinary team. This role will make initial nursing assessments and treatment decisions based on the needs and wishes of the patient and their family. The Admissions and Care Coordinator will also gather important contact information, review and document the patient's medication list, and take their full medical history.

Responsibilities:

The Admissions and Care Coordinator will provide and arrange for hospice care for patients/families in agreement with public health nursing standards, practices, and policies. This role will supervise LPNs, CNAs, Unit Secretaries, and Volunteers. May be asked to rotate to other regions/departments.

Qualifications:

Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred. Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred. Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from a compact state is also accepted. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Must have a driver's license in good standing and access to a vehicle during working hours.
