Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team in Highland Park, IL. As a Physical Therapist, you will provide community-based outpatient services to patients in their homes, working with the geriatric population.

Job Summary:

This is a full-time, part-time, or PRN Physical Therapist position to perform outpatient therapy within home-based settings in the Highland Park, IL area. The successful candidate will have a Doctor of Physical Therapy degree (or equivalent standing) and be eligible for Illinois state licensure as a Physical Therapist.

Responsibilities:
  • Provide community-based outpatient services to patients in their homes
  • Work with the geriatric population
  • Have a minimum of 12 hours of availability per week
  • Be available to work 3 weekdays per week
  • Perform ongoing 1-on-1 direct patient care (for PRN/Per Diem candidates)
Requirements:
  • Graduation as a Doctor of Physical Therapy (or equivalent standing)
  • Eligibility for Illinois state licensure as a Physical Therapist
  • Availability to work a minimum of 12 hours per week
  • Regular and consistent weekly availability (for PRN/Per Diem candidates)
