- Ensure Compliance: Ensure compliance with regulatory standards and company policies and procedures to maintain high-quality care delivery.
- Document Patient Interactions: Accurately and timely document all patient interactions, assessments, and interventions to maintain a comprehensive patient record.
- Current LVN Licensure: Possess current licensure as a Licensed Vocational Nurse (LVN) in the state of California.
- Experience in Care Management: Minimum of 1-2 years of experience working as an LVN in a healthcare setting, preferably in care management or case management.
- Strong Clinical Assessment Skills: Possess strong clinical assessment and critical thinking skills to identify areas for improvement and develop effective care plans.
- Excellent Communication Skills: Demonstrate excellent communication and interpersonal abilities to effectively collaborate with the healthcare team and communicate with patients and families.
- Ability to Work Independently: Ability to work independently and collaboratively as part of a multidisciplinary team to achieve shared goals.
- Computer Skills: Possess proficiency in computer skills and electronic health record (EHR) systems, with Epic experience preferred.
- Commitment to Patient-Centered Care: Demonstrate a commitment to providing patient-centered care and promoting positive patient outcomes.
