Medical Professional | Palliative Care Specialist

We are seeking a highly skilled and compassionate RN Case Manager to join our team at the Karen Ann Quinlan Memorial Foundation. As a key member of our hospice care team, you will play a vital role in providing top-notch patient care and support to individuals and their families during times of need.
Job SummaryThe successful candidate will be responsible for coordinating patients' rights and advanced directives, explaining Medicare Hospice Benefit when indicated, and performing ongoing physical and psychosocial assessments of Hospice patients. Additionally, you will follow established plans of care, report observations to team leaders or primary nurses, and collaborate with interdisciplinary teams (IDT) regarding new nursing diagnoses and changes in plan of care.
Key Responsibilities- Patient Assessment and Coordination: Explain and interpret patients' rights and advanced directives, and explain Medicare Hospice Benefit when indicated.
- Ongoing Assessment: Perform ongoing physical and psychosocial assessments of Hospice patients.
- Plan of Care: Follow established plans of care for each patient and report observations to team leaders or primary nurses.
- Collaboration: Collaborate with IDT regarding new nursing diagnoses and changes in plan of care.
- Patient Education: Instruct and supervise the activities of hospice aides/LPN staff, assist patients/families in obtaining durable medical equipment and supplies essential to care, observe and record signs and symptoms of disease process, communicate with physicians, families, and other healthcare team personnel, and perform patient/family teaching utilizing teaching tools and handouts.
To be considered for this role, you must possess:
- Strong Nursing Skills: Strong nursing assessment and documentation skills, clinical knowledge to support the dying process, ability to serve as a resource to patients and families during times of high stress and volatility.
- Interpersonal and Communication Skills: Excellent interpersonal and communication skills, ability to comprehend spoken and written English language, and communicate with clarity both verbally and in writing.
- Clinical Experience: Minimum one-year adult, pediatric, or medical/surgical clinical experience, with prior home care or hospice experience preferred.
- Licensure: Licensed to practice as a Registered Nurse in the State of New Jersey, with licensure in Pennsylvania if practicing in PA.
