The Department of Pharmaceutical Services at the University of California, San Francisco (UCSF) is seeking a highly skilled Pharmacist to join our team as an Anticoagulation Pharmacist. This role will provide pharmaceutical care to patients at all associated ambulatory care pharmacy locations within the UCSF Health System.
Key Responsibilities- Obtain medication histories and review medication profiles to ensure safe and effective medication use.
- Provide drug information to patients and healthcare providers, and collaborate with clinicians on drug selection and therapeutic plans.
- Ensure access to medications and educate patients on proper medication use, including cautions, adverse reactions, and interactions.
- Participate in an interprofessional team consisting of physicians, advanced practice providers, nurses, and other clinic staff.
- Oversee pharmacy technicians to facilitate insurance authorizations and prepare and dispense prescription medications and pharmaceutical products.
- Doctor of Pharmacy degree (PharmD) from an accredited school of pharmacy.
- Current license in good standing to practice pharmacy in the State of California.
- Basic Life Support (BLS) certification from the American Heart Association.
- Experience in ambulatory care pharmacy, preferably with a PGY1 or PGY2 residency in a relevant area.
