Job SummaryWe are seeking a Licensed Practical Nurse (LPN) to join our team at an outpatient dialysis center. As an LPN, you will play a critical role in delivering care to patients who are often dealing with multiple co-morbidities. You will work with your head, heart, and hands each day in a fast-paced environment.
Responsibilities- Deliver care to patients with end-stage renal disease or chronic kidney disease
- Work collaboratively with the healthcare team to develop and implement individualized care plans
- Provide education and support to patients and their families
- Participate in quality improvement initiatives to ensure high-quality patient care
- Current LPN-LVN license in the state of practice required
- CPR certification required
- IV certification required if required by state regulations
- High school diploma or equivalent required
- Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
