Job Summary:
This position involves serving as a case management role, responsible for an assigned group of patients, typically geographically related. The successful candidate will be accountable for the oversight and overall management of post-acute services in the home setting, providing direct patient care and supervision of LPNs and HHAs in accordance with established plans of care. Key responsibilities include:
  • Collecting data pertinent to the healthcare consumer's health or situation from patient assessments, telephonic contacts, and EMR information
  • Analyzing data to determine issues and available resources to seek effective resolution
  • Prioritizing multi-issue situations appropriate to the criticality of patient health and welfare, patient/staff safety, and impact on timely services
  • Developing and implementing identified plans, directing the care coordination of a multidisciplinary clinical team approach, supervision of LPNs and/or HHAs involved in the care of assigned patient caseloads

Requirements:
To be considered for this role, you must have a Graduate degree from an accredited School of Nursing, BSN preferred. A valid RN license or temporary license as a Registered Nurse Petitioner in the state where you work and reside is also required. Additional education, training, certifications, or experience may be necessary within the department by the nurse leader.
