Job Title: Registered Nurse Home Health

We are seeking an experienced Part-Time Registered Nurse Case Manager to join our team at Medi Home Health & Hospice in West Jefferson, NC.

Job Summary

The successful candidate will provide high-quality nursing care to our home health patients in Ashe County, NC, and be responsible for developing and implementing individualized care plans.

Responsibilities
  • Conduct initial evaluations and ongoing assessments of patients' nursing needs.
  • Develop, implement, and revise patient care plans as necessary.
  • Provide education and training to patients and their families on home nursing procedures and other care needs.
  • Collaborate with healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Current RN licensure in the state of NC.
  • Minimum of one year of nursing experience, with a focus on home health care.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
